Why it matters

Diabetic retinopathy is the leading cause of preventable blindness worldwide

In the Caribbean, where diabetes prevalence rates are among the highest globally, the burden of DR is severe — yet access to ophthalmologists is limited, and most cases go undetected until irreversible damage has occurred.

Regular retinal screening can prevent up to 98% of severe visual loss. The challenge is doing it affordably, consistently, and at scale in primary care settings that lack specialist equipment.

How it works

From clinic to AI result in under a minute

No ophthalmologist on-site required. CRENO integrates into your existing primary care workflow seamlessly.

01

Register Patient

Clinician opens the CRENO mobile app and registers the patient with basic demographic and health history.

02

Capture Fundus Image

Retinal image taken via smartphone adapter and uploaded to encrypted cloud storage over Wi-Fi or mobile data.

03

Record Vitals

Bluetooth sensor captures heart rate, SpO₂, temperature, and estimated blood pressure simultaneously.

04

AI Classification

EfficientNetV2-S model classifies the image into one of five DR severity levels in under 30 seconds.

05

Clinical Action

Doctor reviews the result on the web portal and generates a PDF report with recommendations and referral guidance.

Why CRENO

Four capabilities. One platform.

CRENO combines three complementary technologies into a single, cohesive platform — purpose-built for Caribbean primary care environments with limited infrastructure.

AI Retinal Classification

EfficientNetV2-S deep learning model trained on APTOS 2019 achieves 82% weighted F1 across five DR severity classes. Post-hoc threshold optimization improves Moderate DR recall to 79%. Test-time augmentation further boosts reliability.

Physiological Monitoring

Custom ESP32-based hardware integrates MAX30102 pulse oximetry and MLX90614 infrared temperature sensing via Bluetooth. Blood pressure is estimated from pulse wave features — providing a complete vitals snapshot at the point of screening.

Clinical Platform

React Native mobile app handles patient registration, image capture, and Bluetooth vitals collection. A web clinical portal supports doctor dashboards, patient management, screening history, analytics, appointments, and PDF report generation.

Caribbean Context

Designed for resource-limited Caribbean healthcare

Standard ophthalmology equipment costs tens of thousands of dollars and requires specialist operators. CRENO was designed from the ground up to function on a smartphone, work in clinics with intermittent connectivity, and produce results that generalist doctors can act on immediately.

The Caribbean faces a diabetes epidemic and a specialist shortage simultaneously. CRENO was built to close that gap — starting with sight.

Smartphone-based capture

3D-printed optical adapter turns any smartphone into a fundus camera.

Remote monitoring

Physiological data and retinal history tracked over time. Deterioration flagged automatically.

Faster referrals

High-urgency cases surfaced immediately. Portal generates referral-ready reports.

Population-aware AI

Trained with attention to Afro-Caribbean retinal characteristics where standard tools underperform.

FAQ

Common questions about CRENO

CRENO's EfficientNetV2-S model achieves a weighted F1 score of 82% across five DR severity classes, with a 97% recall rate for Severe and Proliferative DR — the cases where timely referral is most critical. Post-hoc threshold optimization ensures Moderate DR recall reaches 79%, minimizing clinically meaningful missed diagnoses.
CRENO requires a smartphone (Android or iOS) running the CRENO mobile app, a lightweight 3D-printed optical adapter for fundus imaging, and the custom ESP32 Bluetooth vitals sensor module. No expensive ophthalmology equipment is needed. The web clinical portal works in any modern browser.
The mobile app supports offline data entry and image capture. Images are queued for upload and AI analysis when connectivity is restored. Core patient records are cached locally. The web portal requires an internet connection to access the Firebase-backed database and AI backend.
All patient data is stored in Google Firebase with role-based access controls enforced at the database rule level. Retinal images are stored in Firebase Storage with private access tokens. The AI backend is hosted on Google Cloud Platform. Data handling is aligned with HIPAA best practices and Caribbean data protection standards.
CRENO has three role types: Doctors/Clinicians who access the full clinical dashboard; Patients who can view their own results, appointments, and health goals; and System Administrators who manage doctor approvals and platform accounts. All accounts are reviewed and approved before access is granted.

Understanding Diabetic Retinopathy

What your result means and what to do next

What is diabetic retinopathy?

Diabetic retinopathy (DR) is an eye condition caused by damage to the blood vessels in the retina due to prolonged high blood sugar. It is one of the leading causes of vision loss in working-age adults, but when caught early it is highly treatable.

Regular retinal screening is the single most effective way to detect DR before symptoms appear.
Severity levels explained
No DR No signs detected. Continue annual screening and maintain good blood sugar control.
Mild DR Early vessel changes. Monitor blood glucose closely. Screen every 6–12 months.
Moderate DR More significant changes. Tighter glucose and blood pressure control needed. See your doctor soon.
Severe DR Significant retinal damage. Urgent referral to an ophthalmologist is required.
Proliferative Most advanced stage. Abnormal vessels growing. Immediate ophthalmology care needed to prevent vision loss.
How to protect your vision
Control blood sugar
Keep HbA1c below 7% where possible
Regular check-ups
Attend all screenings, even if vision seems fine
Blood pressure
Target below 130/80 mmHg
Take medications
Do not skip diabetes or BP medications
